SMS Consent & Opt-In Workflow

Public 10DLC opt-in evidence page for CCM RPM Services. This page documents every method used to collect express SMS consent and includes all required SMS program disclosures.

1. Brand Identification

CCM RPM Services uses SMS messaging to communicate with individuals who have expressly opted in to receive text messages.

2. SMS Program Description

CCM RPM Services may send SMS messages related to chronic care management, remote patient monitoring, appointment reminders, care coordination updates, service updates, enrollment follow-up, educational information, health check-in notifications, and occasional informational or promotional messages.

3. Consent Is Optional

Consent to receive non-essential marketing, promotional, or informational SMS messages is not required as a condition of receiving services, purchasing any service, submitting a form, completing enrollment, or completing any transaction.

4. Online Opt-In Form Workflow

Below is the actual online SMS opt-in form used on this page. The SMS consent checkbox is unchecked by default and is optional — the form can be submitted without checking it. SMS consent is recorded only when the individual affirmatively checks the box.

5. Paper Form Opt-In Workflow

The same disclosures and the same optional SMS consent checkbox appear on paper enrollment forms used at participating physician practices and at in-person enrollment events. The paper form mockup below shows the exact language used:

For paper forms, the individual must manually check the optional SMS consent checkbox. Staff records SMS consent only when the checkbox is checked. If the checkbox is not checked, the individual is not enrolled in the SMS program.

6. Verbal Phone / In-Person Opt-In Script

Authorized staff use the following script verbatim when collecting verbal SMS consent over the phone or in person:

“CCM RPM Services can send you text messages about chronic care management, remote patient monitoring, appointment reminders, care coordination updates, service updates, enrollment follow-up, educational information, health check-in notifications, and occasional informational or promotional messages. Message frequency varies, typically 1–4 messages per month. Message and data rates may apply. Consent is optional and is not required to receive services or purchase any service. You may reply STOP to opt out at any time or HELP for help. Our Privacy Policy and SMS Terms are available at ccmrpmservices.com. Do you agree to receive these text messages from CCM RPM Services?”

Staff records the individual’s verbal consent only after the individual affirmatively says yes. If the individual does not clearly agree, the individual is not enrolled in the SMS program.

7. QR Code, Flyer, or Advertisement Opt-In Workflow

If a QR code, flyer, brochure, advertisement, or printed material is used, it directs individuals to this SMS opt-in page or to an online form that contains the same required disclosures and an optional unchecked SMS consent checkbox. SMS consent is not collected from the QR code or flyer alone. Consent is collected only after the individual completes the opt-in form or otherwise affirmatively agrees through an approved opt-in method.

SMS consent and phone numbers collected for SMS purposes will not be shared with third parties or affiliates for marketing or promotional purposes. SMS opt-in data is used only by CCM RPM Services and authorized service providers necessary to deliver the messaging service.
