CCM RPM Services uses this opt-in page for informational and transactional SMS messages only. Messages may include inquiry responses, consultation follow-up, enrollment follow-up, appointment reminders, care coordination messages, CCM/RPM program updates, billing or service follow-up, account notifications, and support messages.
This page is not used to collect marketing or promotional SMS consent. If you would like to receive marketing or promotional SMS, you must separately opt in on the Marketing SMS Opt-In page.

Electronic opt-in method
Users opt in to informational/transactional SMS by entering their mobile number and checking a separate, unchecked informational SMS consent checkbox on this page or on a contact, consultation, request-information, or enrollment form.
This workflow collects consent for informational/transactional SMS only. It does not collect marketing/promotional SMS consent.

Marketing opt-in method
Users who want marketing/promotional SMS must separately visit https://ccmrpmservices.com/marketing-sms-opt-in/ and check the separate, unchecked marketing SMS consent checkbox. Marketing consent is optional and is not required to receive services, consultations, enrollment assistance, or informational/transactional SMS.
Paper opt-in method
CCM RPM Services does not currently use paper forms for SMS opt-in. If paper opt-in is used in the future, the paper form will include the same separate consent language and will be retained as proof of consent.
Verbal opt-in method
CCM RPM Services does not currently use verbal opt-in for marketing SMS. If verbal opt-in is used for informational SMS, staff will read the approved consent script and record the consent date, time, staff member, phone number, and consent type.
Opt-in data handling
CCM RPM Services records consent type, name, mobile number, timestamp, source page URL, IP address if available, user agent if available, and the exact consent language displayed at submission. Mobile numbers and SMS consent data are not shared, sold, rented, or transferred to unauthorized third parties or affiliates for marketing or promotional purposes.
