Program Name
CCM RPM Services SMS Program
Informational / Transactional SMS
Informational and transactional SMS messages may include inquiry responses, consultation follow-up, enrollment follow-up, appointment reminders, care coordination messages, CCM/RPM program updates, billing or service follow-up, account notifications, and support messages.
Users opt in through the CCM RPM Services informational SMS opt-in page or through a contact, consultation, request-information, or enrollment form with a separate unchecked informational SMS consent checkbox.
Marketing / Promotional SMS
Marketing and promotional SMS messages may include service announcements, program awareness messages, newsletters, marketing updates, and promotional information.
Users opt in to marketing/promotional SMS only through the dedicated Marketing SMS Opt-In page. Marketing consent is separate from informational/transactional consent.
Marketing SMS consent is optional and is not required to receive informational messages, consultations, enrollment assistance, services, or to do business with CCM RPM Services.
Message Frequency
- Informational/transactional messages: typically 1–4 messages per month.
- Marketing/promotional messages: typically up to 4 messages per month.
Message and Data Rates
Message and data rates may apply.
Opt-Out
Reply STOP to cancel SMS messages. After STOP, no further SMS messages will be sent unless the user opts in again.
