CCM RPM Services, operated by SDAR IT Solutions LLC ("we," "us," or "our"), offers an SMS messaging program ("CCM RPM Services SMS Program") to support patient care coordination for physician practices. Through this program, patients enrolled in participating practices may receive text messages related to appointment reminders, care follow-ups, and health check-in notifications.
Patients may opt in to the CCM RPM Services SMS Program through one of two channels, both requiring freely-given express written consent that is not a condition of receiving medical services:
(1) Online — at https://ccmrpmservices.com/sms-opt-in/, the patient enters their full name and mobile phone number, then ticks an unchecked-by-default consent checkbox containing the full SMS disclosure (brand identification, message types, message frequency, “message and data rates may apply,” STOP and HELP instructions, and links to these SMS Terms & Conditions and our Privacy Policy) before submitting the form.
(2) In person — during clinical onboarding at a participating physician practice, the patient signs a paper enrollment form bearing identical disclosure language and an explicit, separately-initialed SMS consent box. The form clearly states that SMS consent is optional and is not required to receive care.
All opt-ins are stored with a timestamp, source (web or paper), and the exact disclosure text version shown to the patient.
Messages sent through this program are transactional in nature and may include:
Message frequency varies depending on your care plan and scheduled appointments. Typically, enrolled patients may receive between 1 and 4 messages per month.
You may opt out of receiving SMS messages at any time by replying STOP to any message you receive from us. After opting out, you will receive a final confirmation message and will no longer receive SMS messages from this program. You may opt back in at any time by contacting your physician practice.

Message and data rates may apply depending on your mobile carrier and plan. We are not responsible for any charges incurred from your carrier for receiving SMS messages.
Our SMS program is supported on all major U.S. carriers. Carriers are not liable for delayed or undelivered messages.
