SMS Terms & Conditions

Program Overview

CCM RPM Services, operated by SDAR IT Solutions LLC ("we," "us," or "our"), offers an SMS messaging program ("CCM RPM Services SMS Program") to support patient care coordination for physician practices. Through this program, patients enrolled in participating practices may receive text messages related to appointment reminders, care follow-ups, and health check-in notifications.

Consent and Opt-In

CCM RPM Services collects SMS/RCS consent through two separate, unchecked consent options. Patients and prospective practice clients may choose to opt in to one, both, or neither. Marketing/promotional consent is separate, optional, and is never a condition of receiving services, consultations, enrollment, or informational messages.

Channel 1 — Online opt-in: at https://ccmrpmservices.com/sms-opt-in/ and on every contact, consultation, and request-information form, the user enters their full name and mobile number, then independently checks one or both of the following unchecked-by-default consent boxes:

  • Informational / Transactional SMS/RCS Consent — covers inquiry responses, consultation follow-up, enrollment follow-up, appointment reminders, care coordination messages, CCM/RPM program updates, health check-ins, billing or service follow-up, and account or service notifications.
  • Marketing / Promotional SMS/RCS Consent (separate, optional) — covers service announcements, newsletters, promotional offers, and marketing updates. This consent is collected separately and is never required.

The full SMS disclosures (brand identification, message types, message frequency, “message and data rates may apply,” STOP and HELP instructions) and links to the SMS Terms & Conditions and Privacy Policy appear directly above each checkbox before the form is submitted.

Channel 2 — In person: during clinical onboarding at a participating physician practice, the patient signs a paper enrollment form bearing identical disclosure language and two separate, unchecked SMS consent boxes (one for informational/transactional and one for marketing/promotional). The form clearly states that SMS consent of either type is optional and is not required to receive care.

Each consent choice is recorded separately as informational_sms_consent and marketing_sms_consent with the user’s name, mobile number, timestamp, source page URL, IP address if available, user agent if available, and the exact consent text version displayed at the time of submission. Informational/transactional consent is never used to send marketing messages, and marketing consent is never used to send messages outside the disclosed CCM RPM Services SMS/RCS program.

Types of Messages

A. Informational / Transactional Messages — sent only to users who opt in to informational/transactional SMS/RCS. May include:

  • Inquiry responses and consultation follow-up
  • Enrollment follow-up and program enrollment confirmations
  • Appointment reminders
  • Care coordination messages
  • CCM/RPM program updates
  • Health check-ins
  • Billing or service-related follow-up
  • Account or service notifications

B. Marketing / Promotional Messages — sent only to users who separately opt in to marketing/promotional SMS/RCS. May include:

  • Service announcements
  • Newsletters
  • Promotional offers
  • Marketing updates

Marketing/promotional SMS/RCS consent is separate from informational/transactional SMS/RCS consent. Marketing consent is optional and is not required to receive informational messages, services, consultations, enrollment, or to do business with CCM RPM Services.

Message Frequency

Message frequency varies depending on your care plan and scheduled appointments. Typically, enrolled patients may receive between 1 and 4 messages per month.

Opt-Out Instructions

You may opt out of receiving SMS messages at any time by replying STOP to any message you receive from us. After opting out, you will receive a final confirmation message and will no longer receive SMS messages from this program. You may opt back in at any time by contacting your physician practice.

Message and Data Rates

Message and data rates may apply depending on your mobile carrier and plan. We are not responsible for any charges incurred from your carrier for receiving SMS messages.

Supported Carriers

Our SMS program is supported on all major U.S. carriers. Carriers are not liable for delayed or undelivered messages.

Privacy
